Do I Need to Get a New DOT Physical to Apply for a CDL Learner’s Permit?

I am in Pennsylvania so I am primarily interested in that state.

I have a current medical card and everything is good with it. However, in the bottom right corner under “CLP/CDL Applicant/Holder” the “no” circle is selected instead of the “yes” one. I am ready to go and submit my Comercial Learner’s Permit application and was going to use this medical report when I do so (PA requires a medical card to issue a learner’s permit). I was wondering if the “no” selection would be an issue. I cannot find anywhere online the concept of a current medical card being invalid for CDL but maybe I’m not looking in the right place.

To put it simply; can I apply for a CDL learner’s permit with a medical card that says “no” under “CLP/CDL Applicant/Holder”?
